Usage

Fill in as follows:

1. URL of document being referenced to (normally a PDF document)
2. fill in the title e.g. "minutes of Council Meeting on 10 June 2010"
3. fill in request number (appears in Whatdotheyknow.com url after "request/"
4. fill in name of public authority e.g. Department for Culture, Media and Sport
5. fill in access date
